New Patient Packet

Required for every new patient before the first visit. Save 15 minutes at check-in by completing these at home.

    Demographics, contact info, pharmacy preference, and primary-care provider.

Records Release

Use these forms to move records between offices. Sign the one that matches the direction you need.

Fax signed forms to (830) 624-7505 or bring them to your visit. Records requests typically take 7–10 business days to fulfill.

Ask the scheduling team if one of these is needed for your visit — not every patient fills these out.

  • Pre-operative checklist

    For patients scheduled for surgery — labs, medications to hold, and fasting instructions.

How to Complete Your Forms

  1. Download and print.

    Open the PDF, print single-sided, and complete in blue or black ink. If you're completing through the portal, you won't need to print.

  2. Sign every signature line.

    Any missing signature delays your check-in. HIPAA and financial policy forms require a signature even if you're a returning patient with updated information.

  3. Bring or send ahead.

    Bring to your visit, or fax to (830) 624-7505 the day before so we can pre-register you.

If a form won't open, a PDF is too large to print, or you'd prefer a paper packet mailed to you, call the scheduling team. We can email a secure link or mail a printed packet to your address on file.

Confirm whether the practice mails paper packets and how far in advance to request one.
